Dr. Nefario is the main antagonist in the MySims Kingdom (DS) storyline. In the night at the start of the game, a strange man in a black suit, hinted to be Dr. Nefario, sucks up everything in town with a strange contraption. He later attempts to do it again in the mountains, but after sucking up one item, the player has a "!" above their head. Dr. Nefario notices and runs to his UFO, which he sometimes uses to escape his crimes. It is revealed at the end of the game that his secret base is in the alligator cage at Tim's zoo. The player and Mecha-Dog work together to destroy Nefario's base. After catching him, he apologizes for his crimes and reveals he only did what he did to do some sort of research.

- Dr. Nefario's name is based off the word "nefarious", meaning evil, wicked, or villainous.
- Dr. Nefario is also the name of Russell Brand's character in the Despicable Me franchise.
- Despite all the main NPCs in MySims Kingdom (DS) appearing as their silhouettes in the group shot before the player interacts with them, Dr. Nefario's silhouette is never there before the player interacts with him.
